AvondFocus provides online ADHD care as an affiliated practice within De GGZ Coöperatie U.A. Treatment is governed by the General Terms of De GGZ Coöperatie U.A. In addition, the AvondFocus Supplementary Practice Terms apply to our specific way of working in online ADHD care. The legally binding versions of both documents are published in Dutch.
These terms form the legal basis of the care agreement and have been issued by De GGZ Coöperatie U.A., the legal entity under which AvondFocus delivers care. They cover the treatment agreement, medical records, complaints procedure, payments and liability.
In addition to the cooperative terms, AvondFocus applies its own practice terms covering our specific way of working: video consultations, the registration route, the no-show policy and practice-specific arrangements for ADHD care.
